Read the following case study and answer the reflective questions. Please provide rationales for your answers. Make sure to provide a citation for your answers.,"CASE STUDY: An Older Immigrant Couple: Mr. and Mrs. Arahan Mr. and Mrs. Arahan, an older couple in their seventies, have been living with their oldest daughter, her husband of 15 years, and their two children, ages 12 and 14. They all live in a middle-income neighborhood in a suburb of a metropolitan city. Mr. and Mrs. Arahan are both college educated and worked full-time while they were in their native country. In addition, Mr. Arahan, the only offspring of wealthy parents, inherited a substantial amount of money and real estate. Their daughter came to the United States as a registered nurse and met her husband, a drug company representative. The older couple moved to the United States when their daughter became a U.S. citizen and petitioned them as immigrants. Since the couple was facing retirement, they welcomed the opportunity to come to the United States. The Arahans found life in the United States different from that in their home country, but their adjustment was not as difficult because both were healthy and spoke English fluently. Most of their time was spent taking care of their two grandchildren and the house. As the grandchildren grew older, the older couple found that they had more spare time. The daughter and her husband advanced in their careers and spent a great deal more time at their jobs. There were few family dinners during the week. On weekends, the daughter, her husband, and their children socialized with their own friends. The couple began to feel isolated and longed for a more active life. Mr. and Mrs. Arahan began to think that perhaps they should return to the home country, where they still had relatives and friends. However, political and economic issues would have made it difficult for them to live there. Besides, they had become accustomed to the way of life in the United States with all the modern conveniences and abundance of goods that were difficult to obtain in their country. However, they also became concerned that they might not be able to tolerate the winter months and that minor health problems might worsen as they aged. They wondered who would take care of them if they became very frail and where they would live, knowing that their daughter had only saved money for their grandchildren’s college education. They expressed their sentiments to their daughter, who became very concerned about how her parents were feeling. This older couple had been attending church on a regular basis, but had never been active in other church-related activities. The church bulletin announced the establishment of parish nursing with two retired registered nurses as volunteers. The couple attended the first opening of the parish clinic. Here, they met one of the registered nurses, who had a short discussion with them about the services offered. The registered nurse had spent a great deal of her working years as a community health nurse. She informed Mr. and Mrs. Arahan of her availability to help them resolve any health-related issues.  Why was the study done? Make sure the study is directly relevant to the clinical question.

 What is the sample size? Size can and should vary according to the nature of the study.  Are instruments of the variables in the study clearly defined and reliable? Make sure the variables were consistently applied throughout the study and that they measured what the researchers said they were going to measure.

Anna is a 45-year-old female that presented to her physician’s office for her annual check-up. Anna has a history of diabetes, obesity, and noncompliance with diet and medications to control her diabetes. She a single mother of three teenagers and smokes regularly. During the history review, Anna shares with you that she has not been feeling like herself for the past six months, she has been unusually tired and thought that she felt a lump in her right breast during a self-breast exam around that same time. She stated, “I am very busy with my children; I haven’t had time to get it checked out.” She has recently been experiencing right nipple pain. Anna has a positive family history of breast cancer; both her mother and grandmother have been treated for breast cancer. Anna has never had a mammogram. During the breast examination, the practitioner palpated a lump in Anna’s right breast. No discharge from the nipple was observed. Anna’s right breast was tender upon palpation. No abnormalities were found in the left breast. Based on the physical findings Anna will undergo a diagnostic mammogram.

The idea of the future to lessen the illness load of diabetes mellitus (DM) and better the wellbeing of all human beings who have or are at chance for, DM.

(2018), DM type 2 Diabetes Mellitus (T2DM) is reported to affect one in 11 adults worldwide, with over 80% of T2DM patients inhabit in low-income economies. Medical care systems play an essential role in acknowledging this increasing global pervasiveness and are key to protect effective diabetes management by monitoring their glucose level. International Diabetes Federation’s Diabetes Atlas reported in 2015 that 415 million people globally, have diabetes, most with Type 2 Diabetes Mellitus. Even though the rate and generality of T2DM vary by geographic region, with more than 80% of T2DM patients live in low- -income countries, T2DM pervasiveness has expanded worldwide since 1980 but health care play an important roll in the response to this rising energy cost, preventing premature death by eating a portion of healthy food, being a non-smoker and disability and better well being. 

Identify the national-level key stakeholders who are affected by the DM type 2 practice problem and stakeholders involved in the resolution.

Ogurtsova et al. (2015) state, that there is no easy way for directing diabetes but coordinated, a more component to improve health can make a difference. Everybody can play a part in a role in lessening the effect of all forms of diabetes. The health care team, governments, health-care providers including osteopathy, podiatrist, dentist, chiropractor, clinical psychologist, optometrist, nurse practitioner, nurse-midwife, or a clinical social worker, people with diabetes, food producers, civil society, and creator suppliers of medicines and technology the branch of knowledge dealing with engineering or applied sciences are all stakeholders. Altogether, they can make a big contribution to stop the rise in diabetes and improve the lives being supportive individuals of those living with the disease.

Giving excellent quality, evidence-well informed clinical guidelines (CPGs) provide a way of see through the gap between policy, local contexts, best practice, and patient choice in healthcare. Clinical guidelines have been supporting as a crucial part of providing the care the patient needs for several years. Clinical practice guidelines are a suitable way of wrapping evidence and presenting problems to healthcare advocates. Quality advancement initiatives are affiliated with CPGs, as evidence-based guidance forms the basis for combine core results and performance standards (Kredo et al.,2016).
