Nurse’s Role in Influencing the Legislative Process

The first two questions relate to influencing a bill that is traveling through the legislative process. Evidence based practice project

As a nurse in a healthcare organization, it has become evident that patient outcome data have been consistently declining in some patient care settings. Some of the outcomes under inspection include patient falls, (3 % increase in the last 6 months) .

You have been asked to investigate contributing factors to the declining outcomes and participate with a team approach to developing an evidence-based practice project. As you started this exploration, you came across this article Aiken Article titled ""The effects of nurse staffing and nurse education on patent deaths in hospital with different nurse work environment"" , which served as a starting point for the assignment you have been given. You may need to secure some current articles to support you as you develop the clinical question. For example, you may want to retrieve peer review articles on patient Fall.

1. Access the link to the Aiken article above and read the article. This article will provide some background information that links nurse-patient ratios to patient outcomes. The article is provided to get you started on the development of an evidence-based project (EBP).

2. Refer to the Iowa Model (available in the textbook) to serve as a guide in the development of the EBP project. Be sure to explain, in detail, how each step of the Iowa Model supports your project development.

3. Based on the information in the article, develop a PICO question to address the problem of a decline in patient outcomes.

Evidence-Based Practice and the Quadruple Aim

Healthcare organizations continually seek to optimize healthcare performance. For years, this approach was a three-pronged one known as the Triple Aim, with efforts focused on improved population health, enhanced patient experience, and lower healthcare costs.

More recently, this approach has evolved to a Quadruple Aim by including a focus on improving the work life of healthcare providers. Each of these measures are impacted by decisions made at the organizational level, and organizations have increasingly turned to EBP to inform and justify these decisions.

To Prepare:


 Read the articles by Sikka, Morath, & Leape (2015); Crabtree, Brennan, Davis, & Coyle (2016); and Kim et al. (2016) provided in the Resources.

 Reflect on how EBP might impact (or not impact) the Quadruple Aim in healthcare.

 Consider the impact that EBP may have on factors impacting these quadruple aim elements, such as preventable medical errors or healthcare delivery.


To Complete:

Write a brief analysis (no longer than 2 pages) of the connection between EBP and the Quadruple Aim.

Your analysis should address how EBP might (or might not) help reach the Quadruple Aim, including each of the four measures of:

Renal System Case Study

Mr. Drummond is a 72-year-old male who is 2 days postoperative for a bowel obstruction. His urine output over the last 4 hours is less than 100 ml per hour. The MD has ordered Normal Saline IV at 200 ml per hour.

Question 1

The nephron being the functional unit of the kidney, list the three main functions the nephron is responsible for.

Question 2

The MD orders the following diagnostic and laboratory tests: kidney, ureter, and bladder roentgenography (KUB), intravenous pyelography, creatinine and blood urea nitrogen (BUN). Explain how each one can help in the diagnosis of kidney disease.

Question 3

Mr. Drummond has an elevated BUN and creatinine. The MD orders a creatinine clearance. Explain how this is done and what information it will give you to help diagnose Mr. Drummond’s renal problem.

Caring for a pediatric Burn patient

Missy is 3 years old. She enjoys being with mom and grandma in the kitchen. She pulls a chair close to the stove, climbs on the chair, and looks inside the pot of boiling water. The steam is hot on her face, she steps back and as she falls off the chair, the boiling water is pulled down on her. Missy is rushed to the emergency department. Grandma has put some butter on her fingers, but her face was not treated.

Review the Rule of Nines found on page 1239.

In the ED, the nurse discovers that her hands, face, chest, and arms are burned. The Dr. believes it to be a partial-thickness burn, but notes her hands are blistered and very edematous.

Provide nursing care to this young patient that is complaining of pain, crying, and uncooperative. Educate the mother and grandmother on the extent of the burn and the on-going care that will be needed.

Include medication you may wish to request from the physician and the dosage.

Rubric:

1. Fully explains the rule of 9's. What is it? Why is it used? Use it on Missy and share the findings. 

2. Based on the information in the Discussion the student correctly identifies ONE nursing diagnosis (and all required parts) AND identifies ONE goal written in SMART format.
